Dear Parent/Guardian

The Rev Youth Group is excited to host a fun and engaging "Bigger and Better" Youth Night on November 14th! As part of this activity, youth participants will have the opportunity to travel in the motor vehicle of a parent or volunteer in groups to various locations/homes in our community trading up for bigger and better items. If you have any questions please contact Pastor Grace MacLellan at 780-207-5444.

Please read and complete the following consent form to grant permission for your child to participate and to allow travel with other parents or volunteers.

By completing the form below I consent for my child to participate in the Rev Youth Group's "Bigger and Better" Youth Night on November 14th, 2024. I understand that my child may travel in the motor vehicle of a parent or volunteer. I acknowledge that the Rev Youth Group, its leaders, and volunteers will not be held liable for any injury or accident that may occur during this event, including while traveling in the vehicle of another parent or volunteer.
